Anxiety is not only a mental experience. It is a full-body event, and understanding what is happening in your body can take a lot of the fear out of the sensations themselves. The engine behind those sensations has a name you have probably heard: the fight-or-flight response.
Fight-or-flight is your body's built-in emergency system. When your brain perceives a threat, it triggers a cascade designed to prepare you to fight or run. As the American Psychological Association describes, the nervous system signals the adrenal glands to release adrenaline and cortisol, which speed the heart, raise blood pressure, and mobilize energy. Breathing quickens to take in more oxygen, muscles tense ready for action, and blood is redirected to the large muscles. Every one of those changes had a survival purpose in a physical emergency.
Here is the catch that explains modern anxiety. This system evolved for physical dangers, a predator, a rival, a cliff edge, and it cannot tell the difference between a real physical threat and an anxious thought. So when you worry about an email, a diagnosis, or a conversation, your brain flags a threat, and the body mounts the same full physical response it would for a lion. The alarm system is the same. Only the trigger has changed, from a lion to a look, and the body reacts identically.
This is why anxiety produces such vivid physical symptoms: the pounding heart, the shortness of breath, the churning stomach, the trembling, the sweating, the tense shoulders. None of these means something is medically wrong. They are the fight-or-flight response doing exactly what it evolved to do, just in response to a threat that is not physical and cannot be fought or fled.
Understanding this matters because the sensations themselves often become a second source of fear. You feel your heart racing and think something is wrong with my heart, which is a fresh threat, which triggers more fight-or-flight, which speeds your heart further. That loop, sensations feeding fear feeding sensations, is how a spike becomes a panic attack. Knowing that the sensations are your alarm system, not a medical emergency, is what breaks the loop.
And it is genuinely reassuring to know they are not dangerous. The NHS is clear that panic attacks, for all their intensity, are not dangerous, will not cause physical harm, and usually pass within minutes. Your racing heart is your heart working, not failing. The system is uncomfortable by design, because discomfort gets your attention in an emergency, but it is not harmful, and it always winds down.
There is also a built-in off switch, which is the whole basis of the physical calming tools. The fight-or-flight response is run by the sympathetic branch of your nervous system, and it has a counterpart, the parasympathetic branch, which brings the body back down. You can deliberately engage that brake, most directly through slow breathing with a long exhale, which we cover in slow breathing for anxiety.
The other thing to know is that the response is designed to pass. Fight-or-flight is a short-term emergency state, and the body cannot sustain the peak for long, so a spike of anxiety, left alone, crests and falls on its own, usually within minutes. This is why riding out a wave without fighting it works: the wave was always going to recede. Fighting it, or fearing the sensations, is what keeps it high.
One caveat worth naming: when the stress response is switched on chronically, running low-grade all day rather than spiking briefly, it takes a real toll on the body over time, which is part of why chronic overthinking is so draining. We cover that in the hidden cost of overthinking.
The takeaway is oddly comforting: your anxious body is not malfunctioning, it is a superb survival system firing at the wrong targets.
